2012 Theme: How Patient Centred Care Will Change Our World
Conference presentations will address patient-focused care in an optimal health care system. While each profession plays an important role in providing patient-centred care, health care can be fragmented and delivered in silos if there is not a focus on the patient perspective. The conference will provide delegates with insights into the meaning of patient centred care, examples of successful interprofessional integration and how different structures and models affect access and patient care. Learn strategies and develop the tools that will help health care workers thrive in an integrated environment and provide optimal patient centred care. Building on the outstanding success of the 2007 and 2009 inter-professional conferences, the Alberta College of Pharmacists, the Alberta Medical Association, the Alberta Pharmacists' Association, the College and Association of Registered Nurses of Alberta and the College of Physicians & Surgeons of Alberta have once again joined forces to offer this unique program to health care professionals in Alberta. This conference will stretch modes of thinking, stimulate awareness, and provide a call to action so that health professionals will be better equipped and motivated to provide integrated, patient-focused care.
