Below is a list of the videos submitted for the Golden Safety Pin Awards. These files are quite large and are available to download in .zip folders. Depending on your connection speed, it will take time to download most of these files:

Golden Safety Pin Awards: Video Submissions Process

Calling all budding film-makers! We are looking for short videos – no longer than 3 minutes - on topics related to healthcare safety.

Please read this Call for Videos thoroughly before making your submission.

Note: Videos submitted for the Halifax 9 Golden Safety Pin Awards will also be considered in the Canadian video competition for the 2009 pan-Canadian broadcast of the Institute for Healthcare Improvement's (IHI) National Forum. Videos

Videos should document safety initiatives that have taken place or are currently taking place.

The topic of the video should provide insight into human performance issues in healthcare safety, for example, one of the H9 themes. Videographers may also document a healthcare safety theme of their choosing. The video may be aimed at members of the public, healthcare providers, or organizational and regulatory administrators.

Examples of possible topics include communication and collaboration with patients, design, disclosure, hand hygiene, human factors, patient and staff safety, technology, training, safety education, and story-telling for safety.

Submissions

All submissions must include the following information:

  1. Name(s) of videographers
  2. Institution / Address
  3. Title of video
  4. Format (.mpeg or .mov)
  5. Duration
  6. Specific objective
  7. Intended audience (e.g., patients, nurses)

Submission Review Process

All videos submitted will undergo review. The Video Review Committee will assess each submission on criteria that will include:

  1. How well the objective of the video was achieved, based on the stated purpose
  2. Quality and originality of concept
  3. Direction
  4. Adherence to format
  5. Graphics
  6. General appeal

Videos will be judged based on the above criteria and the winner will receive the "Golden Safety Pin" / "l'Epingle d'or de Sécurité" award.

Submission Requirements and Format

All videos submitted will undergo review. The Video Review Committee will assess each submission on criteria that will include:

  1. Videos must be received by August 15, 2009.
  2. Each video must be submitted on DVD-rom to after completing and emailing the submission form (below):

  3. Videos should adhere to one of the following three formats: .mov, .mpeg or .avi.
  4. Videos must not exceed 3 minutes in duration (file size ~100mb).
  5. Videos may be selected for presentation at the Symposium and on the Symposium website. Those that are selected for presentation will be shown unedited, as submitted.
  6. An individual may submit more than one video.
  7. There is no fee to submit video(s).
  8. Any person submitting a video will be responsible for obtaining all relevant releases and permissions and hereby verifies their submission does not violate or infringe on any rights, including but not limited to privacy, publicity or intellectual property rights, or constitutes copyright infringement.

Official Submission Form

The official submission form is in Microsoft Word format and must be completed electronically. If you do not have MS Word, please contact the Abstract Coordinator to make alternate arrangements. To download and submit the official form, please follow these instructions carefully:

  1. Right-click this link and choose “Save Target As...” to save the file to your computer:

  2. Complete the highlighted boxes on the form and double-check that you have answered all of the necessary sections of the form.
  3. Save your file before submitting.
  4. Email the completed submission form to halifax@buksa.com.
  5. An official confirmation of receipt of your file will be forwarded within 48 hours. If you do not receive this official confirmation, please contact the Video Coordinator to be sure your file was received.
