Registration Policies
- Registration fees must be paid in full before you can be admitted to the conference. If there is an outstanding balance on your account when you arrive at the conference, you will be asked for a credit card or cheque to settle your account.
- All registration fees will be subject to 15% HST and are listed in Canadian
Dollars (CAD).
- Invoices must be paid within 30 days of the date raised. After this period, invoices become "past due."
- Advance registrations will be accepted until October 12, 2010. After October 12, 2010, registrations will only be accepted on site.
- Notice of cancellation must be made in writing to the Conference Secretariat
at halifax@buksa.com. A $100 ($115.00 incl. 15% HST) cancellation fee will
apply for conference registrations cancelled until September 13, 2010. After
this date, no refunds will be issued.
- Substitutions may be made until October 12, 2010 by writing to the Conference Secretariat at halifax@buksa.com or on site.
- Credit card payment must be made by VISA or MasterCard. Your statement will read "Buksa Assoc. (780) 436-0983 Edm."
- Cheques must be made payable to "BUKSA Associates Inc." In the event of an NSF (non-sufficient funds) cheque, the individual or organization will be charged a $20 ($21 incl. 5% GST) service fee.
- A confirmation notice detailing your registration and an invoice or receipt will be sent to you within 10 working days of receipt of your completed registration form.
- Completion of this form constitutes a binding agreement to the policies listed above.
Accommodation Room Rates
Accommodation at the Halifax Marriott Harbourfront may be booked at the following rates:
- Standard room: $189.00/night plus taxes
- Upgrade room: $219.00/night plus taxes
Accommodation Policies
- A room block and special rates have been secured at the Marriot Halifax Harbourfront Hotel for conference delegates. All guest room reservations at the Marriot Halifaxt Harbourfront Hotel must be made through the Conference Secretariat.
- Guest room reservations are made on a first-come first-served basis, subject to availability.
- Reservations must be received by September 20, 2010 and are based on availability. After this time, reservations will be subject to general availability at the prevailing corporate rate.
- Reservations must be guaranteed with a major credit card. VISA MasterCard or American Express will be accepted for this purpose.
- Individuals will be required to arrange payment for their guest rooms on arrival at the hotel by credit card. Organizations may arrange for payment of guest rooms by requesting a Third Party Billing form after completing this registration form.
- Reservations may be cancelled up to 72 hours in advance of the check-in date without penalty. Deposits are refundable if cancellation is received prior to October 20, 2010.
- Conference rates are offered three days before and three days after the meeting dates of October 21, 2010 to October 23, 2010, subject to availability.
- Overnight self-parking is subject to a daily charge of $8 per vehicle. Valet parking is subject to a daily charge of $15 per vehicle.
- All room rates are subject to, per adult service charge, 2% Tourism Marketing Levy and the 13% Harmonized Sales Tax.
- Triple accommodation is available at an additional cost of $20 CAD per day. Quadruple accommodation is available at an additional cost of $40 CAD per day. The Marriot Halifax Harbourfront Hotel allows a maximum of four (4) adults per guest room. Children under the age of 18 years, sharing a room with their parent/guardian(s) stay at no charge.
- Check-in time is 4:00 pm. Check-out time is 12:00 pm. You may check in earlier depending on occupancy levels and availability of "ready" rooms.
What to Expect at the Symposia
The Halifax Series has evolved into Canada's flagship event in healthcare safety. Participants from previous meetings have consistently remarked about the meeting being innovative, cutting edge, and intellectually challenging.
The meeting is different by design. In developing the early programs, the founders of the Halifax Series sought inspiration within healthcare and in other industries around the world for different ideas, knowledge, skills and attitudes which would present opportunities for the improvement of healthcare safety in Canada. The Halifax Series Organizing Committee has diligently continued this approach.
