Rooms reservations at the Rimrock Resort Hotel must be made using this form and are available at the following rate:

• Standard: $139.00/night plus taxes

Room rates are subject to availability and booked on a first-come first-served basis.

REGISTRATION POLICIES

1. Registration fees must be paid in full before you will be admitted to the conference. If there is an outstanding balance on your account when you arrive at the conference, you will be asked for a credit card or cheque to settle your account.
2. All registration fees include 5% GST and are listed in Canadian Dollars (CAD).
3. Invoices must be paid within 30 days of the date raised. After this period, invoices become "past due."
4. Advance registrations will be accepted until April 6, 2012. After April 6, 2012, registrations will only be accepted on-site.
5. Notice of cancellation must be made in writing to the Conference Secretariat at PedsCard12@buksa.com. A $105.00 (incl. 5% GST) cancellation fee will apply for conference registrations cancelled until March 20, 2012. After this date, no refunds will be issued.
6. Substitutions may be made until April 6, 2012 by writing to the Conference Secretariat at PedsCard12@buksa.com or on-site.
7. Credit card payment must be made by VISA or MasterCard. Your statement will read "Buksa Assoc. (780) 436-0983 Edm."
8. Cheques must be made payable to "BUKSA Associates Inc." In the event of an NSF (non-sufficient funds) cheque, the individual or organization will be charged a $21.00 (incl. 5% GST) service fee.
9. A confirmation notice detailing your registration and an invoice or receipt will be sent to you within 10 working days of receipt of your completed registration form.
10. Recording may occur through the conference; by agreeing to these policies conference attendees consent to filming, photography and sound recording as delegates, speakers or exhibitors at the conference.
11. All registration rates are subject to availability.
12. Completion of this form constitutes a binding agreement to the policies listed above.

ACCOMMODATION POLICIES

1. A room block and special rates have been secured at The Rimrock Resort Hotel for conference delegates. All guest room reservations at the Rimrock Resort Hotel must be made through the Conference Secretariat.
2. Guest room reservations are made on a first-come first-served basis, subject to availability.
3. Reservations must be received by March 11, 2012 and are based on availability. After this time, reservations will be subject to general availability at the prevailing corporate rate.
4. Reservations must be guaranteed with a major credit card. VISA, MasterCard or American Express will be accepted for this purpose. Individuals will be required to arrange payment for their guest rooms on arrival at the hotel by credit card. Organizations may arrange for payment of guest rooms by requesting a Third Party Billing form after completing this registration form.
5. Reservations may be cancelled up to 96 hours in advance of the check-in date without penalty (one night room and tax charge).
6. Conference rates are offered three days before and three days after the meeting dates of April 10, 2012 to April 13, 2012, subject to availability.
7. Overnight self-parking is subject to a daily charge of $16 per vehicle. Valet parking is subject to a daily charge of $30 per vehicle.
8. All room rates are subject to a $9.00 per day, per room resort fee, a 4% provincial tourism levy and the 5% Goods and Services Tax (GST).
9. Triple and quadruple accommodation is available at an additional cost of $25 CAD per person, per day. The Rimrock Resort Hotel allows a maximum of four (4) adults per guest room. Children under the age of 18 years, sharing a room with their parent/guardian(s) stay at no charge.
10. Check-in time is 4:00 pm. Check-out time is 12:00 pm. You may check in earlier depending on occupancy levels and availability of "ready" rooms.
